I worked at a place where everyone(!!!) had the same login to the windows network. The general account had administrative access. When the NT admin was fired for not backing up a 20tb san, I put proper logins in place since I had to cover both his network and my osx/rasterizing system network.

I was given access to a payment processing server a couple of days ago, with an SU capable login of kenneth and a password of k3nn3th. Guess what the CTO's name is. No, those logins and passwords don't work any more. Did I mention he has a PhD in CompSci? (Name slightly changed to protect the idiots)
